Each week, you will reflect on your time with the teacher candidate using this form. You may choose to complete this form with the candidate or simply review the form with them after you have filled it out.
You will give this form to the teacher candidate upon completion each week. The teacher candidate will store the forms in their binder during the clinical practice phase. Upon completion of the phase, the teacher candidate will submit the forms to the assigned university supervisor via Taskstream.
Each week, you will complete this chart detailing when and how co-teaching was utilized. You will complete this form with the candidate.

You will formally observe three lessons taught by the teacher candidate throughout the phase. Using these formal observations as well as your informal observations of the candidate throughout the clinical practice experience, you will complete three forms (sent as a Taskstream survey via e-mail): Midterm Assessment, and Final Assessment. Before submitting the forms on Taskstream, print a copy for your teacher candidate’s binder for reference during conferences with the candidate and/or university supervisor.
You will be sent this form as a Taskstream survey via email to complete at the end of the phase (Final Disposition). The survey will ask you to reflect on the disposition of the teacher candidate while in your classroom. You may download the form here beforehand to view the questions and possible responses.

If you are experiencing any sort of difficulty with the teacher candidate, the first level of intervention is implemented within the assigned site of clinical practice. This site intervention requires a written action plan (with a designated time for the intervention) and plans for providing extra support.
The action plan specifically identifies the area(s) of concern and the methods for collecting performance evidence during the planned intervention. The action plan will be signed by the candidate, the cooperating teacher and the university supervisor. After the signed action plan is reviewed by the program coordinator, a copy of the action plan will be placed in the candidate’s PLNU file.
You will provide this competency assessment form to the teacher candidate at the beginning of each phase. At the end of the phase, the teacher candidate will submit the competency form to you via Taskstream. By the end of phase 2, the teacher candidate must show competency in each aspect of the competency form.
